Chronic prostatitis / chronic pelvic pain syndrome (CP/CPPS)

Chronic prostatitis / chronic pelvic pain syndrome (CP/CPPS) is a recurring or long-lasting condition caused by swelling and inflammation of the prostate gland, a walnut-sized gland located directly below the bladder in men. The prostate gland produces fluid (semen) that nourishes and transports sperm.1

Symptoms

Inflammation or irritation of the prostate gland can cause pain and discomfort in the lower pelvic region,2 which is experienced by approximately 9% of Canadian men over the course of any given year.3 Up to 50% of men will experience symptoms at one point during their lives.4 Symptoms can include:

  • Pain or burning sensation when urinating (dysuria)5
  • Difficulty urinating, such as dribbling or hesitant urination5
  • Frequent urination, particularly at night (nocturia)5
  • Urgent need to urinate5
  • Pain in the abdomen, groin or lower back5
  • Pain in the perineum5
  • Pain or discomfort of the penis or testicles5
  • Painful ejaculations5
  • Flu-like symptoms (with bacterial prostatitis)5
  • Blood in the urine (hematuria)4


Chronic prostatitis (CP) is the most common urologic diagnosis in men older than age 50 years and is the third most common diagnosis in men younger than age 50 years.6

The upoint phenotype tool

CP/CPPS can present different symptoms and patients frequently have varying etiologies that require multimodal therapy. Clinical trials have shown the efficacy of therapy guided by the UPOINT phenotype,7,8,9 with one study indicating that 84% of men experienced significant improvement of symptoms related to CP/CPPS based on the NIH Chronic Prostatitis Symptom Index (CPSI).7

  • Chronic prostatitis / chronic pelvic pain syndrome (CP/CPPS) may be caused by a bacterial infection, but in many cases the cause of CP/CPPS is unknown. When CP/CPPS is caused by bacterial infection it can be treated with antibiotics. When the cause is unknown, treatment of the symptoms may be the best course of action.
